dc.description.abstract | Transformed Practice is transfer in meaning-making practice, which involves applied learning, real-world meanings, communication in practice, and applying understanding gained from Situated Practice, Overt Instruction, and Critical Framing to a new context. A transformed health care system is required to achieve this goal. Transforming the health care system will in turn require a fundamental rethinking of the roles of many health professionals, including nurses. In health care system today, language terminologies use in documentation of care rendered is needed for improvement in patient care, patient safety and patient outcomes. However, for continuity of care to be maintained within the system, appropriate measures need to be put in place as often times nursing task documentation level has been fragmented, which is obviously noticed with recurring communication barriers and unacceptable levels of errors. Nurse residency programs should be instituted to provide nurses with an appropriate transition to practice and develop a more competent nursing workforce. | en_US |
